Organization: Maple Family Health Team
Description of Services: Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff * one-on-one appointments or group classes available

Additional services:
  • glucose control
  • meter use
  • hypoglycemia
  • nutritional counselling
  • insulin initiation and adjustments with medical delegation
  • after hours clinic

Fees: Free
Eligibility - Population(s) Served: Adults 18 years and older with pre-diabetes, type 1 diabetes and type 2 diabetes
Note: People using insulin pumps should be referred to Hotel Dieu Hospital's Diabetes Education Centre
Application: Health care professionals can refer to Maple Diabetes by faxing a referral form * community members can self refer by calling the Maple Diabetes office
