Supportive Housing Case Manager builds and maintains relationships with tenants, working with them to create individualized support plans to get the resources they seek, develop their life and self-management skills, and successfully maintain housing. The Supportive Housing Case Manager may also work with other BQWCHC clients to coordinate various agencies, providers and services to help the client meet their health and social needs. |
