Eligibility - Population(s) Served: Admission eligibility will be assessed on a case by case basis by the Patient Flow team.
Application: Central Intake Referral receives the completed referral forms which are available on the Providence Care website and are accepted from South East regional hospitals, Ontario Health atHome, and Community Primary Care Providers
