Description of Services: Regular telephone contact offering friendly contact, support and comfort to those who are home-bound, isolated or during the extended absence of the client's family * on a daily, weekly, bi-weekly or monthly basis

Eligibility - Population(s) Served: People aged 60 years of age or older who have addictions related to the aging process, previous illness or injury, permanent or periodic impairment, or physical disability
Adults between the ages of 18 and 59 may be eligible if:
  • they have a physical disability or a debilitating chronic illness OR they are clients of Hospice Quinte ET
  • their family/friends/caregivers are not available or are unable to provide services AND
  • there is a demonstrated need for Community Care for South Hastings services
Application: Call * an in-home assessment is required to register for telephone services
