Program: Case Management - Mental Health

Description of Services: Case Management program provides intensive case management services to individuals living with mental health and addiction concerns.  Support can be provided to clients in their homes, as well as community and office settings to provide strength-based support to promote recovery, wellness and independent living, crisis intervention, assessment and treatment planning in collaboration with clients and other community supports.  Provides specialized care to clients living with dual diagnosis, acquired brain injury and transitional aged youth.

  • CBT, DBT
  • housing support
  • trustee program
