Fees: | Cost will be communicated at the time of booking * in some instances, the cost of the trip may be paid by your social service agency (Ontario Disability Support Program or Ontario Works) * if it isn’t covered by your agency, you will be responsible to pay the full charge or there may be subsidy support available for qualified individuals | ||
Eligibility - Population(s) Served: | If a client is eligible for transportation through an agency, for example Ontario Works Program or Ontario Disability Support Program, that agency will determine eligibility * a client may also be eligible if they are unable to access other transportation because of physical or financial limitations, or if a commercial taxi service is not available in their area, or relatives are unable to provide transportation | ||
Application: | Call or complete the online request form book two to three weeks in advance for wheelchair transportation * the client is notified by telephone once the booking is confirmed | ||
