Description of Services: Community mental health services for people with severe and persistent mental illness that have significant functional impairments, and have continuous, high service needs * support is given in the community rather than in an office, and combines skill teaching with clinical management * 24 hour daily access is made available to registered clients * works with community organizations to maximize client care based on resources available in the community

Fees: Most fees are covered by a valid Ontario Health Card
Eligibility - Population(s) Served: Between the ages of 18 and 65 with severe and persistent mental illnesses listed in the Diagnostic and Statistical Manual of Mental Disorders (DSM IV) that seriously impair their functioning in community living * priority is given to people with schizophrenia and other psychotic disorders
