Description of Services: Visiting Hospice - companionship and respite care providing emotional support to people and their family/caregivers dealing with a life-threatening illness

· Home Visiting/ Caregiver Respite

· Hospital/ Long-term Care Visiting

· Over-the-Phone Support

· One-on-One Grief and Bereavement Support

· Support Groups (see below)

Bereavement Services - trained volunteers provide one-on-one support to individuals and families grieving the loss of a loved one or friend

Bereavement Support Groups
- held in various communities throughout the year where people can share their grief journey in a safe, confidential environment.

Fees: Free
Eligibility - Population(s) Served: Adults 18 years and older diagnosed with a life-limiting or terminal illness and their families or anyone who has experienced the death of a loved one
Application: Call * no referral required for most programs
