Some health services are only for registered clients, including:
  • primary care
  • diabetes care
  • mental health counselling
  • nutritional counselling
  • workshops and group sessions for people with chronic conditions
  • medication management
 
2SLGBTQI+ Community-based in person and virtual health and social support programs and services for Belleville-Trenton’s Two-Spirit, Lesbian, Gay, Bisexual, Transgender, Queer, and Intersex communities and other communities that identify as sexually and gender-diverse (2SLGBTQI+).
 
Community Resources and System Navigation Assistance for individuals and families needing support or advocacy with day-to-day challenges
 
Food Programs Community nutrition programs are scheduled throughout the year * they may include free meal programs, basic cooking skills, understanding nutrition, weight management, and programs for purchasing low cost fresh fruits and vegetables
 
Harm Reduction Services Supports for harm reduction to support people who use substances to reduce the risk of overdose or infection. 
 
High Risk Foot and Wound Care Wound and foot care for people with high risk chronic illness and advanced foot problems * provided at the Centre
 
Managing Chronic Conditions Services Information and skills-based groups and peer-support groups to help people living with chronic pain, diabetes and other chronic conditions * scheduled throughout the year
 
Mental Health Counselling Groups provide psychoeducation, social and self-help opportunities for individuals to work in collaboration with others addressing how to manage mental health symptoms, learn coping strategies and connecting with others whom identify having lived experience
 
Nutritional Counselling Services Information and education from a clinical dietitian
 
Supportive Housing Supportive Housing Case Manager builds and maintains relationships with tenants, working with them to create individualized support plans to get the resources they seek, develop their life and self-management skills, and successfully maintain housing.
 
Thrive program - Support for mothers with addictions Supports for pregnant or parenting women (children ages 0-6) struggling with substance use
 
Virtual Care–Telemedicine Coordinates Ontario Telemedicine Services (OTN) throughout Hastings and Prince Edward Counties
